Welcome to Peterson AFB’s Military Personnel Section (MPS). Thank you for choosing Peterson Air Force Base for your DEERS/ID card needs. Here is information about Common Access Cards (CAC), Military or Civilian Retiree ID cards, and Military Dependent ID cards. CAC/ID CARD HOURS, LOCATION

NOTE – The MPS, including the CAC/ID Card section, will be closed from 12-4:30 p.m. Tuesday, May 22, for Focus Day. One alternate ID Card facility in the local area will be open on Tuesday afternoon. It is located at 302nd Airlift Wing, Bldg. 895, (719) 556-3625. Normal duty hours for MPS, including the CAC/ID Card section, will resume Wednesday, May 23. For questions, call 556-0006 or 556-0007.

Open 7:30 a.m.-4 p.m. Military in uniform have preference Mon.-Fri. 7:30-10 a.m.
Appointments are scheduled in 30-minute blocks from 8-10:30 a.m. and from 1-3:30 p.m.
We are located at 135 Dover Street, Room 1074 in Bldg 350.

Please read the following information prior to making an appointment.

APPOINTMENT INSTRUCTIONS
A scheduled appointment timeframe takes approximately 25 minutes per session. A session may consist of one CAC ID or up to two Dependent ID cards. Please schedule multiple appointments based on the number of ID cards needed. For example, if you have four dependents, you will need to schedule two appointments.

IDENTIFICATION INSTRUCTIONS
All customers must present acceptable identity documents; anyone age 21 and older must have two forms of ID. One must be a valid state or federal government issued picture ID.

How early prior to expiration date can I get a new card?
Normally, CACs are renewed when they are within 60 days of expiration. Dependent/Retiree ID cards are renewed when they are within 30 days of expiration

CAC RESET: INVALID PIN/BLOCKED/LOCKED ONLY
No appointment is needed for a CAC reset ONLY if you received an error stating you have been “BLOCKED” or “LOCKED” or you have an “INVALID PIN.” This action will be conducted as a walk-in service.

LOST OR STOLEN CAC/ID CARDS
ID cards are the property of the United States government, and holders are required to safeguard their ID Card/CAC at all times. Members will need to bring TWO valid, unexpired forms of identification; one must be a photo ID. For active duty military (E-1 through E-7), a letter from the commander or first sergeant must also be presented.

LOST/STOLEN ID CARDS FOR DEPENDENTS
Dependents: Must have TWO forms of unexpired identification. The sponsor MUST be present at time of issue.

POWER OF ATTORNEY (POA)
Needed if your military sponsor is DEPLOYED or on TDY orders that he/she is unable to accompany you for your appointment for any reason. A valid “General” or “Special” Power of Attorney that will allow you to conduct DEERS business on your sponsor’s behalf.

JUST GOT MARRIED? INITIAL ENROLLMENT OF SPOUSE
When adding your spouse to DEERS, the sponsor must be present or the new spouse has a valid POA with the following documents: A valid state or federal government-issued photo ID, original/certified copy of marriage certificate, Social Security card/Verification of Social Security Number Memo from SSA Office with Social Security Number printed. If dual military, spouse MUST be enrolled and listed under both military personnel. If your spouse is a foreign national, you must have the original marriage certificate, valid passport, and birth certificate. All documents need to be certified and translated into English.

NAME CHANGE FOR SPOUSES
According to Defense Human Resource Activity (DHRA), the name may only be changed if the spouse provides at least one identity source document listed on the OMB I-9 form such as a Social Security card or valid Driver’s License reflecting the name change.

DEPENDENT ID CARDS
For dependent ID cards, the sponsor must be present. If he/she is not available, the dependent must have a DD Form 1172, Application for Uniformed Services Identification Card DEERS Enrollment, signed by the sponsor and witnessed by a Verifying Official or Notary Signature and Seal, or a POA, which must be an original or certified copy.

ADDING NEWBORN/CHILDREN TO DEERS
Sponsor must have the following documents for the child: Original Birth Certificate  / Certificate of Live Birth authenticated by the attending physician and Social Security card (when available). If a stepchild is being added, the marriage license is also needed. Memorial Hospital issues a Verification of Birth Worksheet (REVOI-2006) which states “This is not a legal document.” This is NOT authorized to enroll the child into DEERS.

FULL-TIME COLLEGE STUDENTS
All college students 21 years old must bring a letter signed from their school registrar’s office stating that they are enrolled full time in an accredited college in pursuit of an Associate Degree or higher in order to have the dependent benefits extended to the day before the age of 23.

CHILDREN BORN OUT OF WEDLOCK
Female Sponsors: Child’s birth certificate and Social Security Number.
Male sponsors: Court order establishing paternity or a State Voluntary Acknowledgement of Paternity Form, child’s birth certificate, SSN, and legal documents showing paternity was judicially determined.

ENROLLING DEPENDENT WARD
You will need to provide a DFAS letter of approval, court document placing the child in the member’s household for at least 12 months, birth certificate and Social Security card. Please note that all documents must be translated into English and certified.

PARENT ENROLLMENT
You will need to pick up a Dependency Determination Application from Customer Support and apply for an approval through DFAS. Provide the letter of approval at the time of enrollment, service member’s birth certificate, and a valid photo ID of your parent along with their Social Security card.

PARENT-IN-LAW ENROLLMENT
Enrollment process and documents are the same as the Parent Enrollment listed above but instead of Sponsor’s birth certificate you will need SPOUSE’S birth certificate and marriage certificate. Note all documents must be translated into English and certified.

CIVILIAN EMPLOYEES
If this will be your first CAC issued with a new position, you will need to provide your SF Form 50 (Notification of Personnel Action) at the time of issuance along with two valid forms of unexpired identification.
